Role & Responsibilities:
Primary work of Phlebotomist will be:
(1) Visiting HCP clinics identified by Abbott for as per appointment plan created by Abbott based on mapping provided by Abbott team (subject to agreed off days as stated hereinlaterherein later and unless otherwise revised in writing by Abbott)
(2) Introducing self & Bbriefing doctorspatients about the screenings as per Abbott approved script/note and conducting Hyperuricemia tests using the Point of care device.
Note: The screening data / test readings will not be entered into writing anywhere by phlebotomist
(3) Daily reporting to their supervisor on the activities conducted in the Service Provider's internal CRM application. Post each health clinic, Phlebotomist should share the report (number of patients screened and consumables utilized) with concerned Abbott TBM/ABM as per Abbott shared format
(4) Conducting minimum threetwe health clinics every day in morning and evening OPDs of HCP clinics as per (1) above, excluding on agreed of list public holidays
(5) At the option of HCP and concerned patient, Phlebotomist will assist HCP/patients in using the Abbott's Risk Assessment Scale with each consenting patient in health clinic from www.hyperuricemiaundercontrol.com. Service provider shall ensure the correctness, sanctity, confidentiality and integrity of the report to patient and HCP.
