Job Description: Nurse - Chronic Patient Care Team
The Nurse in the Chronic Patient Care team Clinical Capability Center (CCC) is integral to delivering exceptional healthcare to patients with chronic conditions. This role serves as a liaison between patients and healthcare providers, ensuring effective communication, accurate information exchange, and coordinated care to enhance patient outcomes. The nurse will address clinical queries, manage patient concerns, and provide ongoing support to guarantee optimal care.
Key Responsibilities:
Patient Care:
● Ensure timely and accurate placement of medication orders in the pharmacy, based on precise dosage calculations, strength, and quantity requirements, in accordance with the provider's prescription approval.
● Collaborate with healthcare teams, including US-based doctors and nurses, to assess and ensure continuity of care.
Clinical Communication & Collaboration:
● Maintain seamless communication with US providers, facilitating the smooth transfer of patient information and ensuring the coordination of care.
Nursing Documentation & Compliance:
● Adhere to US nursing standards, protocols, and regulations, ensuring compliance with HIPAA and other legal frameworks for patient confidentiality and privacy.
Patient Communication:
● Engage in therapeutic communication with patients via telemedicine platforms, addressing patient queries and providing regular updates on their care.
Continuing Education & Training:
● Participate in training programs to stay updated on nursing protocols and the latest US healthcare standards.
● Contribute to quality improvement initiatives aimed at optimizing remote care and identifying operational or clinical enhancement areas.
● Assist with the onboarding and mentorship of new team members.
Preferred Qualifications:
● Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN).
● Minimum of 6 months to 1 year of hands-on experience in reputed hospital ICUs or wards.
● Experience with telemedicine, proficiency in Electronic Medical Records (EMR) systems, or remote patient monitoring is preferred.
Work Environment:
● Location: Bangalore (providing remote support for US patients).
● Schedule: Rotational shifts, including night/day shifts, weekends, and holidays, to align with US time zones and operations. Shift duration:12 hrs
