Job Summary:
The Home Health Care Manager/Coordinator oversees patient care services, ensures compliance with healthcare regulations, and coordinates with healthcare professionals, patients, and their families. They manage care plans, schedule services, and ensure that patients receive high-quality, compassionate care in their homes.
Key Responsibilities:
Patient Care Coordination:
Assess patient needs and develop individualized care plans.
Coordinate home health care services, including nursing, therapy, and personal care assistance.
Serve as a primary point of contact for patients, families, and healthcare providers.
Monitor patient progress and adjust care plans as needed.
Staff Supervision & Scheduling:
Recruit, train, and supervise home health aides, nurses, and other caregivers.
Ensure proper staffing levels by scheduling and assigning caregivers to patients.
Provide support and guidance to the care team, ensuring adherence to best practices.
Regulatory Compliance & Documentation:
Ensure compliance with state, federal, and industry regulations, including Medicare and Medicaid guidelines.
Maintain accurate records, including patient assessments, care plans, and service logs.
Conducted quality assurance checks and reported any management concerns.
Communication & Collaboration:
Work closely with physicians, therapists, and other healthcare professionals to coordinate care.
Educate patients and families on care plans, medications, and available resources.
Address concerns, complaints, and emergencies in a timely and professional manner.
Qualifications & Skills:
Education: Bachelor’s degree in Nursing (BSN), Healthcare Administration, or a related field (preferred).
Experience: 2+ years in home healthcare, case management, or care coordination.
Licenses/Certifications: RN or LPN (preferred), Certified Case Manager (CCM) (a plus).
Strong understanding of home health care policies, Medicare/Medicaid regulations.
Excellent organizational, problem-solving, and interpersonal skills.
Proficiency in healthcare software and electronic health records (EHR).
Work Environment:
Primarily office-based with some travel to patient homes and healthcare facilities.
Requires flexibility for occasional on-call duties.
