Epoch Elder Care was founded in 2012, as a home care provider to meet the growing needs for elderly care and services in India. Epoch Elder Care focused on companion care, psychological support & home management. Epoch offered services in Delhi-NCR, Mumbai and Pune to over 400 seniors.
Roles and Responsibilities
Clinical Nursing Procedures:
Conduct weekly vital sign checks.
Perform daily body checks.
Prepare and administer medications, including insulin.
Perform dressing changes.
Manage catheter bag and catheter changes.
Administer oxygen therapy when required.
Staff Supervision:
Supervise both care and non-care support staff.
Monitor staff performance and escalate any issues to the Facility Manager (FM) as needed.
Resident Care & Facility Operations:
Ensure high-quality care for all residents and suggest process improvements.
Cover for the FM in their absence.
Escalate resident feedback, staff concerns, and facility-related issues (e.g., maintenance, housekeeping, laundry, dietary) to the FM.
Notify the FM of required resident care supplies and facility-related items.
Maintain medication indent and order resident supplies as needed.
Family & Communication:
Respond to family inquiries and escalate concerns to the FM.
Provide updates on doctor visits via email.
Inform families about critical health changes and hospitalizations.
Draft updates for the FM regarding facility and resident matters.
Resident Admission & Transition:
Collaborate with the Sales, Transitions, and Care teams to plan and welcome new residents.
Assist in preparing rooms and ensuring all documentation is ready for new residents.
Conduct full admission assessments and complete required paperwork.
Care Plan & Health Management:
Assist the FM in reviewing and updating care plans.
Identify new care requirements for residents (e.g., need for physiotherapy or physician review) and suggest changes to the FM.
Emergency & Incident Management:
Handle emergency situations and assist the FM in root cause analysis.
Complete and submit incident reports to the FM.
Notify family members in case of any incidents.
Documentation & Compliance:
Maintain documentation in the company’s care management software.
Ensure Care Assistants (CAs) follow documentation guidelines.
Print and distribute essential care forms, including shift care forms, intake/output sheets, and hygiene checklists.
Hospitalization & Appointments:
Prepare resident rooms for hospital readmissions.
Ensure hospital discharge instructions are followed by the care team.
Visit residents during hospital stays (both inpatient and outpatient) as needed.
Arrange transportation and personal assistance for routine hospital appointments.
Activities & Engagement:
Develop and manage the monthly activity calendar.
Plan and implement new activities for residents.
Training & Staff Development:
Provide training, support, and guidance to new care staff.
Offer performance feedback on new staff to the FM.
Conduct staff training and retraining sessions as required.
Facility Operations & Meetings:
Monitor staff performance periodically and provide feedback to the FM.
Prepare agendas and facilitate sections of the monthly staff meetings.
Document and distribute minutes of meetings.
Facility Maintenance & Hygiene:
Identify and resolve maintenance issues promptly.
Supervise housekeeping to ensure the highest standards of sanitation and hygiene.
Train support staff to adhere to hygiene and cleanliness guidelines.
Client Interaction & Tours:
Meet with clients and conduct facility tours in the absence of the FM.
Inventory & Vendor Management:
Manage housekeeping and stationery stock.
Oversee vendor management and procurement processes.
